Abstract:
Purpose:
Blepharoptosis with poor levator function is not only a functional problem which strongly associated with
amblyopia in young kids, but also an essential issue in facial aesthetics. Tensor fascia lata sling
procedure is the classic, textbook-standard procedure for blepharoptosis with poor levator function
correction. In recent years, new techniques were developed including FOOM flap and conjoint fascial
sheath suspension, it has become a hot topic which procedure can make better results.
Here, we reported 18 lids of 10 patients with poor levator function blepharoptosis correction using tensor
fascia lata sling procedure combine with upper blepharoplasty.
Materials and Methods:
From March 2017 to March 2022, 10 patients who underwent surgical treatment for blepharoptosis with
poor levator function were collected. Inclusion criteria: Levator function less than 3mm. Surgical
method: Tensor fascia lata 10 to 15 cm harvest from thigh with two, 2 cm incisions, a single pentagonal
configuration of the suspensory sling with fascia lata, using a 1 cm central brow incision and an eyelid
crease incision. An additional upper blepharoplasty is performed for more aesthetic outcome.
Results:
Of all 18 lids of 10 patients, 2 had unilateral and 8 had bilateral ptosis, two of 8 bilateral ptosis patients
had asymmetry ptosis of each eyelid, average age when operation: 39.11 years old (16 – 76 years old),
three patients had received previous ptosis correction surgery. Average follow-up time: 6 months. All
patients achieve good and natural results, with the degree of ptosis less than 2mm and are satisfied
about their appearance. The average eyelash-to-eyebrow distance from mid-pupil has decrease to 1.42
cm postoperatively from 2.35 cm preoperatively. No prolonged lagophthalmus more than one week
happened in this series.
Conclusion:
According to our result, tensor fascia lata sling procedure is an optimal choice in poor levator function
with severe or complicated blepharoptosis patients, which can achieve reasonable results.
Eyelash-to-brow distance has decreased from 2.35 cm to 1.42 cm after surgery. Prolonged
lagophthalmus is not seen in any case in our series. The reason why I prefer tensor fascia lata sling
procedure in this group of patients is that the toughness, stable and long-lasting effect of fascia lata
which can correct more precisely, overcome the scaring tissue of previous operation. Also, it's an
autologous tissue that can last for longer without complications such as infection or protruding. In
combination with upper blepharoplasty surgery, more natural, aesthetic result can be achieved.
