Topic: Personal Technical Tips for Successful Rhinoplasty
Abstract:
In this lecture, I will discuss some of my original techniques in rhinoplasty. To better define and project
the nasal tip, I like to use double layer tip grafting. Modified vertical dome division technique is also one
of versatile and effective way of tip refinement especially when the patient has thick nasal skin. For
dorsal augmentation. My preferred techniques are use of crushed septal cartilage, diced ear cartilage
with perichondrium, and glued diced costal cartilage. Shaping of the glued diced cartilage can be easily
performed using Jang cartilage mold. In surgical correction of saddle nose, proper reinforcement of
septal L-strut is critical. This can be achieved with combination of two extended spreader graft
sandwiching one caudal septal extension graft. In extreme situation, placement of integrated dorsal
implant and columellar strut can provide adequate support to ensure better function and shape. Short
nose is one of the most difficult deformity to deal with. In this case, management of contracted and
inelastic skin is the key element for success. Cartilage flap technique is useful maneuver for
elongating the lateral compartment. Cutting and suture of the L-strut and modified extracorporeal
septoplasty facilitate straightening of the deviated L-strut, thus allowing the surgeon to achieve a
desired aesthetic goal.
